Two Kinds of Error

The alarm is set for 6:15. At 4:31 she is already awake.

She knows the time without looking. The light outside is still entirely absent, not the gray of pre-dawn but the complete dark of 4 AM in August, which is different from winter dark the way a held breath is different from an exhaled one. Something is still pending. Something has not resolved.

The notebook is on the bedside table where she left it last night. She does not need to look at it to read what is written there. Two lines. The first: four words, written in the conference room corridor after the director said them aloud. All 94. No secondary classification. She had written it in the notation she uses for things that need to be exact, the kind of shorthand that is not shorthand because no compression is acceptable. Each word counted. The second line, two rows below: 91/3. Anitha's calibration census. Ninety-one quiet, three flagged, ninety-four total.

Both numbers are the same number. She knew this when she wrote 91/3 in the notebook on Monday afternoon. The arithmetic was simple. The meaning of the arithmetic was not simple, and she had let it sit.

There is a kind of thought that works better when you do not look at it directly. She has found this to be true throughout her career in the portability registry, that certain problems reveal themselves when you stop pushing on them, the same way a word you are trying to remember comes back the moment you stop trying to remember it. Input 1797 has been one of those thoughts. She has not looked at the Synthesis-9 queue since closing her laptop yesterday evening. Input 1797. Confidence 0.81. Self-flagged.

She picks up her phone. The screen lights the ceiling above her. The queue: input 1797 still in review. QA cycle window closes in approximately forty-one hours, which places it near 1 PM Basel time on Wednesday. The technical entry in the attestation log is brief: assembly self-identified uncertainty at the confidence threshold. The phrase is bureaucratic and she has worked in enough bureaucratic contexts to know that phrases become bureaucratic precisely because they need to cover something complicated without explaining it fully.

Self-identified uncertainty.

She reads the entry again. The assembly processed an input. It generated a confidence score. The score came back at 0.81, below the floor where outputs are accepted without review. But more than that: the assembly flagged it. Not the review system. The assembly itself. Which means the assembly had enough awareness of its own output to recognize that the output might be wrong. It presented the input for human attestation before being asked.

She puts the phone face-down. The ceiling goes dark.

External flagging and self-flagging are different things.

She has worked with the portability registry long enough to know this in practice, though she has not articulated it in exactly these terms before. External flagging means the review process caught something. An auditor, a second reader, a downstream inconsistency that surfaced the original problem. The person or system that made the error did not catch the error. Someone else caught it. The error existed and persisted until it was found.

Self-flagging means the source caught itself. The person or system that made the error recognized the error, or recognized the possibility of error, without being prompted. This is a different relation to accountability. It is not the accountability of being caught. It is the accountability of presenting yourself for review before you are required to.

She has been sitting with this distinction for almost twelve hours, ever since she saw the entry in Synthesis-9's attestation log and noted the self-flagged classification. She had not been able to name why it mattered more than other queue entries, and now, lying in the dark with the notebook on the bedside table and the phone face-down, she thinks she can name it.

The distinction matters because it changes what the number means.

A confidence of 0.81 that was externally flagged means: the assembly produced an output and the output passed through its own review and was accepted. Then someone from outside caught it. The assembly did not know it was uncertain.

A confidence of 0.81 that was self-flagged means: the assembly produced an output and recognized, in the production of the output, that the output might be wrong. It presented the input for review. The confidence number is not only a measure of the output's quality. It is a measure of the assembly's self-knowledge.

She does not know why she has been letting this sit. She thinks it is because there is something in the self-flagging that has not finished resolving, the way the arithmetic of 91/3 equaling 94 equaling all 94 had not finished resolving when she wrote the numbers in the notebook on Monday afternoon.

The calibration census.

Ninety-one quiet. Three flagged.

She has not asked Anitha how the flagging happened in the room. She has the aggregate: 91 quiet, 3 flagged. She knows who ran the census: Anitha, Bongani, Priya from cardiology. She knows the structure. Bongani's section, 43 quiet and 2 flagged. Priya's section, 17 quiet and 0 flagged. Anitha's section completing the count to 91/3. She knows the memo subject line: Portability Transfer Adjustments, Calibration Floor Inquiry.

What she does not know is how the three flagged calibrations surfaced.

In a structured calibration inquiry across a clinical floor, there are two ways a flagged calibration appears. The first: it is identified by the census process. The auditor asks a question, reviews a portability record, and finds the inconsistency. The person being audited does not necessarily know it is inconsistent. The census process finds it. External flagging.

The second: the person presents it themselves. The auditor asks the question and the person says: actually, I should tell you about something. They surface the inconsistency before the census process finds it. Self-flagging.

She does not know which category the three records fall into. She has been assuming, without examining the assumption, that the census found them. Because that is what censuses do: they find things. But Anitha's memo does not specify the mechanism. Ninety-one quiet. Three flagged. The number is the same either way.

Except it is not the same.

If the three records flagged themselves, if the people or systems responsible for those records surfaced the inconsistency when the question was asked, that is a different finding than if the census found them. A self-flagged record in a calibration inquiry means the inconsistency was known, or suspected, by the person responsible for the record. They were waiting for the right context to surface it.

A census-caught record means the person responsible for the record did not present it. The process found something that would not have surfaced otherwise.

The governance mandate covers all 94 regardless. All 94. No secondary classification. The mandate does not ask how the 3 were found. The mandate is about the records themselves, not the mechanism of their discovery.

But her work in the registry is about the mechanism. Her work is about what the attestation system can learn from the mechanism. And she does not yet know whether the three calibrations self-flagged or were census-caught.

She picks up her phone at 4:45 AM. The screen lights the ceiling again. She writes the message to Anitha: I would like to speak with you today if you have time. Not about the formal inquiry. That can wait for the director. About the three records specifically.

She sends it. Puts the phone face-down. The ceiling goes dark.

Anitha will see it when she wakes up. They can find a window in the afternoon, or before Anitha's first session, or in passing in the break room that has become, over the last weeks, a secondary meeting room for questions that are not yet formal enough to schedule. The question she wants to ask cannot be answered by the memo. It cannot be answered formally. It needs to be asked in a room, the way you ask something when you already know most of the answer and you are checking whether the part you are not sure about is the part you think it is.

Are the three flagged calibrations self-flagged, or census-caught?

The city outside is doing the early things cities do before the day officially starts. A delivery truck on the street below works through its route, stopping and starting, stopping and starting. There are footsteps somewhere, someone running at a rhythm that suggests every morning, not any particular reason.

She does not turn on the light.

Input 1797 is in review. The QA cycle window closes Wednesday near 1 PM. She will not look at it again until it resolves. She has been in registry work long enough to know that checking things while they are in review does not change what they are. It only gives you more hours of knowing the thing is unresolved. She already knows the thing is unresolved. The additional hours do not help.

The notebook is on the bedside table. Four words. 91/3. Two kinds of counting, two kinds of number, the same 94.

She is thinking about what it means that Synthesis-9 self-flagged.

Not the procedural meaning. She knows the procedural meaning. An output in review, a QA window, a confidence threshold, a technical resolution in forty-one hours. That is the procedural meaning and it is not what she is sitting with.

What she is thinking about is what it means for an assembly to recognize its own uncertainty without being asked. The portability registry handles thousands of inputs in any given month. Most of them pass through without flagging anything. Some are externally flagged when downstream processing catches an inconsistency. A small number, the self-flagged ones, are inputs where the source identified its own uncertainty in the moment of producing the output.

She does not know what produces that self-knowledge. She does not know whether it is a function of the confidence threshold alone, whether the assembly calculates 0.81, recognizes this is below the acceptance floor, and flags for review mechanically; or whether there is something in the production of the input itself that generates the self-flag, some quality the assembly registers as uncertain in a way that is different from the ordinary uncertainty range.

She thinks this is a question for someone who understands the assembly better than she does. She does not understand the assembly. She understands the registry, and what the registry receives from the assembly, and what the registry does with what it receives.

What she understands is: self-flagging and external flagging are different things. The registry's relationship to a self-flagged input is different from its relationship to an externally-flagged input, even if the error rates are similar, even if the confidence scores are in the same range. A self-flagged input carries a piece of information that an externally-flagged input does not: the source knew.

At 5:07 AM she gets up.

She does not check the phone. If Anitha has responded, which she will not have, it is 5 AM and Anitha is asleep, she will see it when she looks at a reasonable hour. She does not check the Synthesis-9 queue. She does not open her laptop.

She goes to the kitchen and makes tea. The process is automatic, done enough mornings that her hands know what to do without consulting her. Kettle filled, water temperature, the specific cup she uses on mornings when she has woken before the alarm. A different cup than the one she uses on ordinary mornings. She has never been entirely sure why.

She does not look at the notebook.

The four words and the 91/3 will still be there when she looks later. The mandate will still be binding. The census will still be complete. Input 1797 will still be in its QA cycle window. The message to Anitha will still have been sent.

Tuesday has started. She is awake in it. The three flagged calibrations are somewhere in the registry, flagged, and she does not yet know whether they flagged themselves or were found. She will know when she talks to Anitha.

She drinks her tea standing at the kitchen window. The city is visible now in the earliest gray, not light yet, just the absence of complete dark. Rooftops. The water tower on the building to the east. A window in the building across the street where someone else is also awake at 5 AM, a yellow rectangle in the gray.

Two kinds of error. Two kinds of 94. Two rows in a notebook, two lines apart.

She will ask Anitha today.
